This section contains specific instructions on how to complete
the Online Police Report. It would be a VERY good idea
for you to print these pages on your printer so that you can
refer to the instruction sheet as you complete the Online
Crime Report Form.
Most of the boxes on the report form are self explanatory. Please
complete all fields that apply to the incident. Much of the
information that you provide is utilized in preparing crime
statistics. As with other documents, an online report
submitted to our agency is subject to open records requests.
WHAT YOU
CAN AND CAN'T REPORT
You CANNOT use this on-line form to
report:

Any type of violent crime, including: personal
assault, kidnapping, robbery, etc.

Any type of sex crime, including: rape, sexual
assault, etc.

Any type of stolen vehicle, including: trucks,
cars, trailers, motorcycles, etc.
-
Any lost/stolen license plates.
-
Any type of traffic accident, whether there was
an injury or not.
-
Any type of court order violation such as
violation of restraining order, violation of child
custody order, etc.
-
Lost Cell Phones (If you have lost your cell
phone you should make a claim with your cell phone
service provider. Any reports received concerning lost
cell phones will not be assigned a report number and
will be destroyed and not kept.)
You CAN use this on-line form to
report:

criminal mischief

thefts from a vehicle

run (no injury)

burglary of a house, garage or business

fraudulent use of lost or stolen credit cards,

Incidents submitted on-line are reviewed to insure that a
crime has been committed before a report number is assigned.
If it is determined that a crime has not been committed, a
report number will not be assigned. If the incident is
classified as a crime and assigned a report number, the
report will generally mailed to you 10-14 business days
after being submitted on-line. If you are reporting an
incident that is not classified as a crime, you will be
mailed a copy of the incident.

OCCURRED OUTSIDE OF THE CITY OF SAGINAW, PLEASE
CONTACT THE APPROPRIATE AGENCY TO MAKE YOUR
REPORT.

GENERAL INSTRUCTIONS
Classification - What type of crime are your reporting?
Currently you may only report the following incident types:

mischief or damage done to your

Car Burglary - Someone
unlawfully enters a locked
vehicle and takes property.

Harassing Phone Calls -
Immediate hang-ups, obscene
language, etc.

Burglary of Home or Business
- Intentional illegal
entering of a structure to take
property of another.

Financial Crime - theft or
fraudulent use of checks, credit
cards, or fraudulent use of your
identity

Other - Crimes that do not
fit any of these categories, but
do NOT involve any kind
of violence or personal attack.
What are the victim's first, middle and last
names?
This is a required field.
If you are the victim, complete the
information as appropriate. If
you are reporting this incident on
behalf of a child, use your
information here, but include their
information in the narrative section. If the victim of a
crime is a business, provide your
information, but indicate that
victim is a business in the
narrative section. Include the
business name.
What is the victim's gender? This is a required field.
Male or Female.
What is the victim's birth date?
This is a required field.
Please enter the
victim's birth date in the form
"mm/dd/yyyy".
What is the victim’s address?
Please list the actual mailing
address of the victim of the crime -
not the location where the incident
occurred. If the victim utilizes a
Post Office Box for receiving mail,
the actual street address of the
victim should be entered.
What are the victim’s phone numbers and email addresses?
Please enter a valid phone number or
numbers. If the victim was a
business, only a business number
needs to be entered. In similar
fashion, the e-mail address should
be that of the actual victim. If a
business is the victim and it has no
e-mail address, you should enter
your e-mail address (as the
reporting person).
When did this incident occur? (Date and Time)
If you know the specific date of the
incident, enter that date using the
following format: month/day/year. If
the incident occurred over a period
of time you may bracket the date in
the following format: month/day -
month/day/year. You may also enter a
time range, or "Unk." If you don't
know what time the incident
occurred. You may also enter
"daytime" or "nighttime."
What is the address or specific location
of the incident?
This is a required field.
List the address of the location of
the incident that you are reporting.
If you do not know the address, you
may describe the location by name if
that location is a commonly
recognized landmark. If the crime
occurred at a residence, list the
address of that residence. If the
incident occurred along a street,
you may give the nearest cross
streets as the location. We must
have an exact location of some kind
to accept your crime report (you
cannot just say "K Street").

Please provide a brief description of the incident:
This is a free form box
in which you can type a
brief explanation of
what happened. Please be
brief and to the point
but give us enough
information so that we
can tell exactly what
happened. An example of
a good description would
be: "I went to the
grocery store at
approximately 1:30pm. My
car was left unattended
in the store parking lot
for approximately 30
minutes. When I came out
I noticed that the left
rear tire had been
slashed". A bad example
would be: "Someone
slashed my tire."
Damaged items?
Include the items
description and a fair
market value for repair
or replacement.
Include how the item was
damaged (if known).
When did you last know the item(s) was/were free of damage?
List date and time if
known.
When did you discover the damage?
List date and time if
known.
What was stolen or lost?
List the items stolen or lost. If this
section is not applicable, enter N/A in the blocks.
Description(s) of property:
There are certain pieces
of information that are
needed when describing
property that has been
stolen or lost. Enter
the data in the labeled
columns. If you do not
know the information for
a category, enter
"Unknown". To enter a
second item, hit the tab
key to repeat the
process in the next
row. If you need more
space, please use the
"More Property" button
at the bottom of this
section.

Indicate whether
boys or girls,
speeds, wheel size
and color.

type of metal (ex:
gold, silver) size,
type and number of
stones (if any),
whether man’s,
woman’s, or child’s
jewelry and any
inscriptions.

whether AM/FM radio,
CD player, Tape

Deck, Walkie-Talkie,
size, and any
additional features.

STEREO EQUIPMENT:
List all components,
speakers, headphones
and the type of
cabinet's. Attach
pictures or drawings
if available.

TELEVISIONS:
Indicate whether
color or black &
white, screen size,
console or portable.
When did you last know the item(s) was/were present?
List date and time if
known.
When did you discover the item(s) missing?
List date and time if
known.
Harassing calls.
Provide specific
information as to times,
what was said, number of
calls, etc.
Do you have any suspects?
If yes, list
all the information you know about the person(s). This can
include (Name, Race, Sex, Date of Birth, Address, Telephone
Number, etc.). Why are they a suspect?

Was there a vehicle involved?
If the crime
occurred to
a vehicle
(such as
vandalism),
there is a
suspect
vehicle, a theft
occurred
from a
vehicle, or
parts were
taken off of
the vehicle,
please complete the
appropriate
fields. If
the vehicle
involved is
yours, you
MUST
include the
license
plate
number.

Submitting a
false report
is
punishable
as a crime
